Referral Partners

A good referral includes
-
participant or client name and contact preferences
-
participant or authorised-representative consent
-
suburb and requested commencement timeframe
-
required tasks, frequency and outcomes
-
relevant access, communication, sensory and safety information
-
funding and plan-management details where applicable
-
referrer and invoicing contact details
Referral Process
-
1. Submit the referral form.
-
2. Goldilocks checks service fit, location and current capacity.
-
3. We contact the participant, authorised representative or referrer as consent allows.
-
4. We clarify scope, risks, schedule, funding and price.
-
5. If everyone agrees, we issue the relevant quote or service agreement.
What helps referrals succeed
Be specific about the functional outcome rather than only naming a support category. For example: weekly support to maintain kitchen and bathroom hygiene and complete laundry, or three organising sessions to create an accessible pantry system.
